Low literacy and limited English proficiency are barriers to health and healthcare for many adults. This Health Information Toolkit is designed to provide health literate culturally and linguistically appropriate information on diabetes, high cholesterol, high blood pressure and a balanced diet. Videos and brochures are available in multiple languages including subtitled versions for those who are hard of hearing.
