If you are employed by a Georgia public or private school system:
You must complete the following forms:
Georgia PSC Certification Application
Employer Assurance Form
You must obtain:
Georgia State University transcript that shows your degree completion

Submit your Georgia PSC Certification Application, your Employer Assurace Form, and your GSU transcript to your school system office. The system personnel director will sign the Employer Assurance Form and submit your materials to the Georgia Professional Standards Commission (it is best to do this rather than you delivering the materials to the Georgia PSC, as your system can verify completion of course requirements so that any pay raise may immediately go into effect. Also, your system might pay the fee for a new certificate.
If you are not employed by a Georgia public or private school system:

Submit your Georgia PSC Certification Application and your GSU transcript directly to the Georgia PSC.
