| Elements to be Included in the FBA | Points |
| Background Information about the Student: age, placement, relevant information about previous achievement, behavioral, and sociological information. | 3 |
| Reason for FBA: Provide information as to how student's behavior might be affecting the achievement of the student or other students. | 2 |
| Primary Hypothesis: Includes antecedents, behavioral statement, and suspected consequences (reinforcers the maintain behavior). Elements in hypothesis must be stated in terms that clearly communicate classroom realities. | 5 |
| Evidence of direct assessment of hypothesis: Assessment assesses hypothesis, is based on direct observation of students behavior, uses appropriate data-collection procedures, and is implemented correctly. Multiple direct assessments are used: Application of direct assessment across multiple settings. | 5 |
| Direct assessment results are interpreted and used to plan subsequent assessments. | 5 |
| Direct assessment results are supplemented by appropriate indirect assessments (interviews, permanent records, etc.) | 3 |
